By participating in the MAPA Mentorship Program, you agree to the following: 

Purpose 

This program supports professional growth, guidance, and connection among Physician Assistants (PAs), PA students, and early-career professionals. All interactions are for educational and developmental purposes only. 

No Professional Advice

Information shared during mentorship is for general guidance only and does not constitiute medical, legal, financial, or other professional advice. It should not replace consultation with a qualifies professional. 

No Formal Relationship

Participation does not create a provider-patient, supervisory, employment, or advisory relationship. 

Personal Responsibility

Mentees are responsible for their own clinical, academic, and professional decisions. Mentorship should not be the sole basis for patient care or clinical judgment. 

Confidentiality & Conduct

Participants agree to maintain confidentiality, communicate professionally, and avoid sharing any protected health information (PHI) or identifiable data. 

Voluntary Participation

Either party may end the mentorship at any time without penalty. 

Limitation of Liability

Participants agree to release and hold harmless the program and its organizers from any liability related to participation.

Acknowledgment

By participating, you confirm that you have read, understand, and agree to these terms.
